Our Mobile Dental Clinic provides dental education and/or dental services to the following schools on a regular basis:

Liberty Elementary School
Livingston Manor Elementary School

Tri-Valley Elementary School
Kenneth L. Rutherford Elementary School
Roscoe Central School
During the summer and school vacations, our Mobile Clinic is open in the parking lot at our offices at 25 Sullivan Ave, Liberty, NY 12754.

We accept Medicaid and most dental insurance plans.
Application Form
Parents/guardians can complete the request form when they bring their child for the dental visit or send it to PRASAD CDHP through the school nurse.
